Update Social Security Number Form Information
Please note that you will be required to provide the following documents:
- Government issued photo identification
- Social Security Card (card must be signed)
Additional information may be required during the review process.
During peak processing please allow up to 10 business days for the submission to be reviewed after all requested documents have been received.
