FORM NAME    INFORMATION

Student Federal Tax Return Transcript

Submit a copy of your (and/or your spouse's) 2011 federal tax return transcript from the IRS.  You can order online by going to www.irs.gov.

Parent Federal Tax Return Transcript

Submit a copy of your parent(s)' 2011 federal tax return transcript from the IRS. You can order online by going to www.irs.gov.

Parent's W-2's

Submit copies of your mother's/father's 2011 W-2's.

Student (and/or Spouse) W-2's

Submit copies of your (or your spouse's) 2011 W-2's.

Copy of HS Transcript Needed

Need to submit a copy of your high school transcript, diploma, GED scores or the equivalent.

Selective Service

According to Selective Service records you are not registered as required by law.  You must submit proof of Selective Service registration or a Status Information Letter.  Additional information may be requested by the Financial Aid Office.

Student Loan in Default

Our records indicate that you are currently in default on a federal student loan.   You must resolve the default with your lender and provide a default clearance letter verifying that you are no longer in default.

Need Bank Statement

Submit a copy of your bank/financial statement(s).

Proof of Current Active Duty

Submit documentation that you are currently serving on active duty in the U.S. Armed Forces for purposes other than training.

Social Security Citizenship

Need documentation/proof of your U.S. citizenship which includes a copy of your U.S. birth certificate, U.S. passport or naturalization certificate.  

Proof of Dependent(s)

Provide documentation that you have dependent(s) who live with you and who receive more than half their support from you, as of now and through the end of the school year for which you are applying for financial aid.

Need Copy of Death Certificate

Need a copy of your mother's and/or father's death certificate.

DHS Verification

Need documentation/proof of your US citizenship or eligible non-citizen status.

Copy of Divorce Documentation

Submit divorce documentation for you (or your parents?).

Proof Emancipated Minor Status

You need to submit a copy of a court's decision from your state of legal residence that you are or were an emancipated minor.

Exceeds Loan Limits

Your total student loan amounts exceed federal loan limits.

 

Proof of Legal Guardianship

You need to submit a copy of a court's decision from your state of legal residence that you are or were in legal guardianship.

Need Marriage Certificate

Need a copy of your marriage certificate.

 

Need Further Information

Submit further information regarding:

Proof Orphan/Foster Care/Ward

Need documentation indicating that both your parents are deceased or you were in foster care or you were a ward of the court at any time since you turned age 13.

Parent Conflicting Tax Info

Your parents' tax information conflicts with your FAFSA data - resolution needed.

Parent's Drivers License

Need a copy of your mother's and/or father's driver's license.

FAFSA/ISIR Reject

The information you submitted on your FAFSA was rejected by the federal processor. Please correct and resubmit to the Dept. of Education.  You can also call the Financial Aid Office at (760) 773-2532 for additional information.

Student Driver's License

Need a copy of your driver's license.

Student Educational Plan

You need to complete a Student Educational Plan (SEP) developed in consultation with a COD academic counselor which lists all course requirements and the timeline for achieving your educational objectives and submit a copy to the Financial Aid Office.

FAFSA/ISIR Signature Needed

Need student/parent signature(s) on the enclosed ISIR Signature Page. Contact Financial Aid Office or go to www.fafsa.gov and follow instructions for you and/or your parents signing the FAFSA.

Social Security

Need documentation/proof of your correct Social Security Number. As such, bring your Social Security card, driver's license and birth certificate or U.S. passport or naturalization certificate to the Financial Aid Office for verification.

Academic Transcript(s) Needed

Need academic transcript(s) from colleges previously attended. Contact the Financial Aid Office.

Unaccompanied Youth/Homeless

You will need to submit a copy of an official determination that you were designated as an unaccompanied youth who was homeless or at risk of being homeless at any time on or after July 1, 2011.

Veteran Affairs Status

Submit documentation/proof of your veteran status (DD-214, etc.)

Loan Entrance Counseling 2012-13

Instructions for completing  Loan Entrance Counseling and Master Promissory Note (MPN):

  • Go to www.StudentLoans.gov
  • Your Federal student Aid PIN (same as used for FAFSA) is required to sign in.  If you have forgotten your PIN go to www.pin.ed.gov to request it.
  • Use the links to Loan Entrance Counseling and Master Promissory Note to complete each item.
  • Your quiz results from Loan Entrance Counseling and your MPN will be sent electronically to your school.  
  • Please allow 10-15 business days for loan updates and disbursements.

Loan Exit Counseling 2012-13

Instructions for completing loan exit counseling:

  • Go to www.StudentLoans.gov
  • Your Federal student Aid PIN (same as used for FAFSA) is required to sign in.  If you have forgotten your PIN go to www.pin.ed.gov to request it.
  • Use the link to Loan Exit Counseling and complete all information.
  • A record of your successful completion of the Loan Exit Counseling will be sent electronically to your school.
Please allow 7-10 days for your school records to be updated.
